Ground breaking diabetes education booking platform

National audit data (2016) showed uptake of diabetes education is poor (8.3% of eligible adults with type 2 and 6.3% with type 1 diabetes). Barriers include poor healthcare unprofessional marketing, lack of choice (venue, date, type of course) and low motivation to prioritise health. In 2018 Health Innovation Network (HIN) coordinated a bid for transformation funding for diabetes education across 12 Clinical Commissioning Groups (CCGs), led by Southwark CCG.

A digital-first approach

Taking a digital-first approach, the PDH platform was deployed to deliver an online booking system covering 12 CCGs, now two ICSs, in south London under the Diabetes Book & Learn brand – also created by PDH.

Pulling data from the patient record at the point of referral and making it available to the education course providers, patients are able to book and attend any course of their choice safe in the knowledge that their data is secure and ready for the provider when they start their course.

And course providers are able to create, edit and manage their own courses through the platform from start to finish.

 

Results

Since launching on 1 September 2018 the PDH platform has been powering the integrated diabetes education booking service for south London, covering a population of 3.2m.
